This site is currently a work in progress but I will try to add helpful documents and links that pertain to our children’s health as well as school health compliance.

IMPORTANT QUICK FACTS

• A physical exam is required for entering PreK, Kindergarten, 1^st, 3^rd, 5^th and 7^th grades.  If your child has their physical anytime during the preceeding year – ask for a copy! 
• A doctor’s note is required for a student to sit out of PE class- if they are seen by doctor or hospital for an injury or illness please ask for a note with guidance regarding recommendations for their return to school and sports
• Please let the school know if your child will be absent for any reason! 
• If your child is prescribed any medications that they may need during school hours a doctor’s order is needed- this document can be found below under “Medication request form”
